De transplantatie van fecale microbiota is een techniek die sinds kort in Frankrijk wordt toegepast voor de behandeling van terugkerende darminfecties met Clostridium difficile bij patiënten met immuundeficiëntie. Niettemin bestaan er nog enkele grijze zones en moeten de klinische grenzen van deze techniek nog worden vastgelegd.
Een mooi bewijs daarvan is deze studie uit New York, waarin wordt aangetoond dat de transplantatie de werking van de hersenen beïnvloedt. De bevindingen, uiteengezet in het tijdschrift eLife, suggereren dat bepaalde combinaties van darmbacteriën stoffen produceren die de samenstelling van de myeline aantasten en die gedragsproblemen kunnen veroorzaken.
Anderzijds geeft dit nieuwe inzicht in de communicatie tussen de microbiota en de hersenen hoop op nieuwe behandelingen voor myelineaandoeningen, zoals multiple sclerose (MS).
De rol van de microbiota
De ‘microbiota’ of darmflora is een verzamelterm voor de micro-organismen in het darmkanaal. De microbiota heeft genen en een eigen metabolisme. Omdat dit de levenskwaliteit van elk individu beïnvloedt, is het belangrijk om te begrijpen welke rol de microbiota speelt.
De microbiota heeft in eerste instantie een metabolische functie, omdat ze de resten afbreekt die niet werden verteerd door de dunne darm. De afbraak van deze bestanddelen (koolhydraten en eiwitten uit onze voeding) zet de productie van verschillende metabolieten in gang.
Talrijke wetenschappelijke studies hebben het belang aangetoond van de microbiota voor de ontwikkeling en vervolmaking van ons immuunsysteem. De microbiota vormt een natuurlijke barrière tegen tal van bacteriën uit onze omgeving, maar beïnvloedt ook ons centraal zenuwstelsel. Heel wat neurotransmitters die inwerken op ons humeur, worden gegenereerd door bacteriën uit ons darmkanaal, en vormen onderzoekspistes om het effect van de microbiota op onze hersenen te verklaren.
De voordelen van een transplantatie
Bij een transplantatie van de fecale microbiota wordt een stoelgangstaal van een gezonde donor via een coloscopie of lavement ingebracht in het darmkanaal van een patiënt met een door Clostridium difficile geïnfecteerde darm. De voordelen van deze techniek werden al in verschillende studies gedocumenteerd. Enkele landen passen de methode vandaag al regelmatig toe, en de transplantatie zorgt bij 90% van de patiënten voor een volledig herstel van de darmfunctie.
Deze studie toont echter aan dat specifieke combinaties van darmbacteriën stoffen kunnen genereren die de samenstelling van de myeline kunnen aantasten en sociaal vermijdingsgedrag kunnen uitlokken, in dit geval bij de muis.
Onderzoekers van de Mount Sinai School of Medicine transplanteerden fecale bacteriën uit de darm van muizen met gedragsstoornissen naar genetisch verschillende muizen met normaal gedrag.
Hun onderzoek toonde aan dat een transplantatie van de fecale microbiota volstaat om sociaal vermijdingsgedrag uit te lokken, de genexpressie te wijzigen en de samenstelling van de myeline in de hersenen aan te tasten bij de ‘ontvangende’ muizen. Myeline, het isolerende omhulsel rond de axonen van de zenuwcellen, is cruciaal voor de werking van de hersenen. Wordt de myeline beschadigd, dan wijzigt de synaptische transmissie en duiken er talrijke klinische symptomen op.
Hoop voor de behandeling van multiple sclerose
Specifieke combinaties van darmbacteriën volstaan dus om stoffen te genereren die de samenstelling van de myeline aantasten en gedragsproblemen uitlokken.
Anderzijds suggereren deze resultaten dat sommige psychische ziekten of zelfs myelineaandoeningen, zoals multiple sclerose, zouden kunnen worden behandeld met gerichte acties op bepaalde bacteriekolonies in de darmen.
“Onze resultaten zullen meer inzicht bieden in de rol van de microbiota voor de modellering van multiple sclerose”, aldus dr. Casaccia, hoofdauteur van de studie. “Onze studie is een principebewijs voor de capaciteit van bepaalde darmmetabolieten om de samenstelling van de myeline te beïnvloeden, ongeacht het genetische profiel van de muis.”
Nieuw inzicht in het communicatiemechanisme tussen de hersenen en het darmkanaal
De onderzoekers identificeerden tevens de darmkolonies verantwoordelijk voor een verhoogd gehalte aan cresol, een stof die door de bloed-hersenbarrière heen kan dringen. Gecultiveerde voorlopercellen van myeline die worden blootgesteld aan cresol, verliezen echter hun capaciteit om myeline aan te maken. Dit doet vermoeden dat deze darmmetaboliet de aanmaak van myeline in de hersenen beïnvloedt.
Kortom, dit zijn cruciale conclusies op vele vlakken: voor de beperkingen van een transplantatie met fecale microbiota, voor het effect van de microbiota op de hersenen en de myeline, en voor de hoop op nieuwe behandelingen met bijvoorbeeld probiotica voor myelineaandoeningen.
